About the Position

The Chief of Population Health & Performance Services at Legacy Community Health reports directly to our CEO regarding overall strategic direction and the coordination of population health, care coordination management, value-based care, and outcomes. You will work directly with Legacy's leadership teams to identify opportunities to implement strategic plans that shift focus from fee-for-service care to value-based care. This will be accomplished by identifying key market drivers and strategies that will address clinical and cost trends with a focus on developing population health that manages the clinical and utilization needs of risk-based patient populations.

  • Medical Doctorate (MD) or Doctor of Osteopathic Medicine (DO)
  • MHA, MBA, MPH, or RN preferred
  • 10+ years of healthcare administration experience
  • Value-Based Care experience
