About the Position

The Manager of Care Management at Legacy Community Health will lead a team of Nurses, Social Workers, Behavioral Health Professionals, and non-licensed staff in the delivery of care management services to Legacy patients. As the Manager, you will oversee day-to-day care management operations and play an integral role in the development and launch of Legacy's value-based care initiatives. Your role will be essential to ensuring timely patient outreach, optimizing care management processes, setting and implementing standards of care, and providing leadership and support to the team.

Key Responsibilities

  • Provide daily oversight and monitoring of all care management activities

  • Ensure discharged patients are identified through daily HIE and MCO reports and are assigned to the appropriate care team member for timely telephonic outreach according to stated guidelines

  • Review internal and external data reports to identify and refer patients who may benefit from care management services to the appropriate care team member for engagement

  • Lead the development and implementation of departmental goals and objectives, policies, procedures, and performance standards

  • Provide consultation, guidance, and direction to all members of the care team

Requirements

  • Registered Nurse with active, unrestricted Texas licensure
  • State Board recognized nursing education
  • 3-5 years of acute nursing experience
  • 4-5 years of utilization management, care management, discharge planning, or value-based care experience
  • 3-5 years of clinical leadership experience
  • Experience with all patient populations, including Commercial, Medicaid, and Medicare
