S:US is looking for an experienced Social Worker to serve as the Outreach Specialist at our Wellness Works Clinic

About the Position

The Outreach Specialist at S:US tracks and monitors all referrals and intakes by adhering to agency standards of required documentation in regard to assessments, consents, screening tools, progress notes, and other intake documentation. You will assess all new admissions for Medicaid eligibility while you maintain compliance with billing targets and Medicaid documentation. As the Outreach Specialist, you will identify problems in daily living skills as well as provide hands-on assistance or referrals in addressing those problems. The Outreach Specialist also serves as the liaison to community-based services and groups, including civic, social service, government, and business organizations.

Salary: $60,000 - $65,000 / year

Requirements

  • Licensed Master Social Worker
  • Able to provide proof of vaccination against COVID-19 virus
  • 2 years of experience with individuals with mental illness, substance abuse issues, and/or history of homelessness
  • Bilingual, preferred
