The Harris Center is looking for an experienced Care Coordinator to join our team providing Mental Health services to Harris County
Shift Schedule: Weekends, 6pm - 6am
About the Position
The Care Coordinator at The Harris Center performs clinical assessments and develop recovery plans by collaborating with the treatment team and service provisions of The Harris Center. You will need to coordinate, link, and monitor the patient and the appropriate services to ensure the consumer's recovery goals are being met. To fully meet the needs of the patient, you will need to deliver interventions, such as skills training; psychosocial rehabilitative services; case management; and medication training, monitoring, and supports.
